Compliance documentation
Each year students are required to complete compliance documentation pertaining to their degree. This includes but is not limited to:
UTS MyPlacement Forms
- Student Inherent Requirements Form
- First Aid Details Form (Nursing only)
- Working with Children’s Check Form (Nursing Only)
NSW Health Requirements
- NSW Health Code of Conduct Agreement for Students
- Undertaking/Declaration Form
- Turberculosis (TB) Assessment Tool
- Blood Borne Virus Declaration (Midwifery Students Only)
- National Police Check (we recommend Veritas)
- Vaccinations (see vaccination checklist)
Start preparing your evidence and obtain any vaccinations needed once you accept your offer and enrol in your first clinical subject.
Further instructions are provided upon your admission to the course.
Students that aren’t compliant by the timelines indicated will not be able to attend placement/s and be at risk of failing the relevant subject. It is the student's own responsibility to ensure their checks are up-to-date throughout their degree.
You may also need
- Evidence of TB screening. This is dependent on how you answer questions in the Tuberculosis (TB) Assessment Tool. See Appendix 3: TB Assessment Decision Support Tool here.
- An overseas criminal record check and or Statutory Declaration Form

Applying
Apply for an Australian Accredited HLTAID011 First Aid Course
A number of course providers provide same day training certification, these include but are not limited to:

Applying
Apply for this check via the NSW Office of the Children’s Guardian.
Step 1
- Complete the online application form as soon as you have accepted your offer and enrolled in a clinical subject
- Apply as a volunteer (If you apply as a Volunteer there is no application fee)
Step 2
- Visit a Service NSW Office to verify your identity. Your application can't be finalised until you do this step. See a list of documents they accept.
Step 3
- You will receive your check within 4 weeks
What if my placement doesn’t involve working with children, do I still need to apply for a check?
Yes! Why?
- You won’t know if your placement needs WWCC, when you do it will be too late to apply
- You will work with children at some point during your degree
- The check is valid for 5 years
- If you don’t have it before your first placement, you won’t be able to attend a clinical placement
