Hypertension, as an intermediate risk factor of cardiovascular disease, remains poorly controlled. In terms of chronic disease management, such as in hypertension, the roles of community-based program are regarded to have had a positive impact on care, particularly in poorly resourced populations. To deepen understanding about how such program plays a role in improving rational use of anti-hypertension medication in the elderly this thesis will elaborate factors underlying self-medication of hypertension and low adherence of hypertension treatment.
