Forms for AusAID students

As an AusAID sponsored student, you are required to complete and submit the relevant forms to the UTS International Sponsored Team if you wish to have access to your scholarship entitlements as listed below:

2. Bank Account details form for stipend commencement (PDF, 152kB)
3. Confirmation of receipt of AusAID establishment cheque (PDF, 128kB)
4. Permission to release information and sponsor information (PDF, 147kB)
5. Supplementary academic assistance (PDF, 109kB) (also referred to as Tutorial Assistance) - provided when essential to improve student performance to ensure academic success.
6. Tutorial assistance monitoring and feedback (PDF, 120kB) – student
7. Tutorial record (PDF, 108kB)- for student and tutor to sign
8. ADS or ALA family invitation verification letter request form (PDF, 141kB)
9. ALA SEA usage application form (PDF, 144kB) - SEA provides financial assistance of up to A$2000 each year to ALA students who commenced studies in Australia prior to 2012, to enhance formal studies.
10. Fieldwork or reunion travel booking form (PDF, 141kB). Use this form for the reunion airfare entitlement available to some eligible students OR fieldwork - once only return economy class airfare via the most direct route may be approved for eligible research Students to their country of citizenship or within Australia.
11. AusAID award extension application form (PDF, 146kB).
